When you receive your test, do the following on your answer document
Mark the correct letter or answer for the missed questions
Copy the statements of the questions you missed and the correct response (not just the letter)
 

On a separate piece of paper (it may be typed)

Title: Standard 6 Evaluation (on the title line if writing on lined paper)
Name and Period on the top corner of the evaluation paper

1.  Write down the standard subset.
2.  Write out the questions you missed with the correct response.  This might require you to copy a diagram or chart along with your answer. Highlight the correct response.
3.  Write down why you  missed the question.
